Make a Clinic Appointment
The UCSF Parkinson's Spectrum Disorders Center welcomes new patients as well as returning patients.

If you already have a diagnosis and physician from the Memory and Aging Center or the Movement Disorders and Neuromodulation Center, please continue to manage your care through your current healthcare team, who will coordinate with the Parkinson’s Spectrum Disorders Center.

New patients may request an evaluation from the Parkinson’s Spectrum Disorders Center at either clinical location. If your symptoms are more cognitive or behavioral, or if you have a diagnosis of progressive supranuclear palsy (PSP), corticobasal syndrome (CBS), dementia with Lewy bodies (DLB) or vascular parkinsonism (VaP), please contact the Memory and Aging Center. If your symptoms primarily affect movement, or if you have a diagnosis of Parkinson’s disease (PD), Lewy body disease (LBD) or multiple system atrophy (MSA), please contact the Movement Disorders and Neuromodulation Center.

The first step to becoming a new patient at the Memory and Aging Center Clinic is to contact our office at 415.353.2057 or have your physician fax a referral to 415.353.8292. Please include the UCSF Medical Center referral form and UCSF Memory and Aging Center Clinic Referral Checklist in the referral.
